ESL Attendance Policy

Coming to class is very important! Please be aware of the ESLP attendance policy:

Students must attend classes and be on time. Students cannot be absent more than 4 times from one class or more than 12 times total for the entire semester.

The Office of International Programs has recently implemented a new International Student and Scholar Portal. You can now submit all OIP request forms online via the OIP's International Student and Scholar (ISS) Portal below. To access the ISS Portal, login with your Seton Hall Credentials.

OIP Requests

To make an OIP request, please log into the ISS Portal and then go to #4 OIP Requests.

Below is a list of OIP Request Forms you will find in the ISS Portal. View each form details by clicking below.

 Change of Level Form
Use this form when beginning a new degree program at Seton Hall. A new I-20 with your new degree information will be created for you. Please submit new finances and items from the Financial Documents Checklist, as needed to support your request.
 Curricular Practical Training (CPT) Request
Use this form when you are offered with an internship and need to apply for work authorization to work off campus. You should discuss with your academic advisor. For more information, please look at the CPT Application Instructions.
 Extension of Program Request
This form is required for all international students who plan to extend their program of study at Seton Hall University. Bring this form to your academic advisor for review of your request to extend. Once complete, please submit this form and supporting financial documents to the Office of International Programs at least 15 days before your program end-date listed on the I-20.
 F-1 Student Contact Information Update
Students must notify OIP within ten (10) days of any change to your physical address or contact information. This is a requirement of the U.S. Government.
 F-1 Student Reduced Course Load Form
In order to maintain your status as an F-1 Visa Student, you must be enrolled at Seton Hall University as a full-time student. If you must become a part time student under unforeseen circumstances, please complete this form for part-time enrollment consideration.
 Graduate Student Full-Time Equivalency Form
Graduate students who have completed all credit hour and curricular requirements except completion of the thesis or dissertation may register for Thesis Continuation or Dissertation Advisement to maintain matriculation status according to the academic policies of the individual school or college. As a result, master's or doctorate student in this situation should submit a Graduate Student Full Time Equivalency Form to OIP with confirmation from the academic advisor that the student is doing full time equivalent work during specific semester if the particular semester is not student’s last semester of study. For last semester of study, students may submit a F-1 Student Reduced Course Load Form.
 Letter Request Form
This form is required for current students to request letters from the OIP. These letters include SSN Letters.
 OPT Documents Submission
Students who have recently been approved for Optional Practical Training and already received an Employment Authorization Document (EAD) Card or I-797A Form USCIS. By submitting a copy of the EAD or approval notice, OIP advisor can double check students’ SEVIS records to ensure OPT work authorization information is accurate listed.
 OPT Recommendation 1-20 Request
Use this form to begin the first step of the Optional Practical Training (OPT) application process by request an OPT Recommendation I-20 before submitting an I-765 OPT application to the U.S. Citizenship and Immigration Services (USCIS). This form can be used for both post-completion OPT and Pre-Completion OPT request.
 Reprint I-20 Request (Including Travel Signature Request)
Request an I-20 from OIP if your current I-20 is updated, lost, stolen, or if you need a new travel signature. Travel signature for students during study is valid for one year from when it is signed.
 Second Academic Major Advisor OPT Confirmation
Use this form if you are applying for an OPT recommendation I-20 with OIP and will be graduating with a double major. OIP needs confirmation from both major academic advisors. This request must be submitted in conjunction with the OPT Recommendation I-20 Request.
 Transfer Out Request
Students who plan to transfer their SEVIS record from Seton Hall to another U.S. Institution should complete this form to make the request. Graduating student should submit this request no later than within 60 days after the program end date. The earliest day OIP can transfer the SEVIS record to the new school is the day after the last day of final exam.
