
Documentation Criteria and Forms
Students requesting an accommodation(s) based upon disability must complete the Accommodation Request and Documentation form (link below).
Students are strongly encouraged to use the electronic form as it is secure for the sharing of medical information. If for any reason a student or provider cannot use the electronic form, a downloadable PDF version of the same form is available below.

Accommodation requests for Housing, Parking and Dining must be submitted each year for which they are requested. Accommodations for Housing, Parking and Dining are reviewed annually and are dependent upon request for and approval of accommodations each year.
Accommodation requests for Academics/classes do not have to be resubmitted each year. However, should the student’s disability or medical condition change, or should the student and medical provider recommend review of approved accommodations, the Accommodation Request and Documentation Form does need to be resubmitted to detail the change.

Housing Accommodation Request
Housing Accommodation Requests must be submitted using the above form by the dates posted on this website in the spring of year preceding the academic year of the request.
Any approved housing accommodation will be assigned by Residence Life on a space-available basis. Housing Accommodation requests submitted after the posted deadline dates will continue to be reviewed; however, approved housing accommodations will be on a space available basis.
Students requesting a housing accommodation must also complete the standard Seton Hall University housing application through the Residence Life portal and pay the housing deposit by the deadlines posted on the Residence Life homepage.
Request for Housing
For Academic Year 2025-26 the deadline has passed. Deadline dates for the 2026-27 Academic Year will be posted in February 2026.
Emotional Support Animal Accommodation Request
An Accommodation request for Emotional Support Animal (ESA) requires completion of an additional separate form available below due to information required about the animal. The ESA Request Form is in addition to the above form.
Any approved accommodation will be limited by space available in the Residence Halls.
Emotional Support Animals are limited to common domesticated animals and to approval by DSS.
ESA Accommodation Request form must be submitted by date indicated on the website. Requests submitted after that date will be reviewed, however approved accommodations will be limited to space available.

Parking Accommodation Request
A request for Parking Accommodation requires completion of the above form.
Review of Parking Accommodation Requests requires information about the location of and frequency of appointments necessitating access to a vehicle.
Dining Accommodation Request
A request for Dining Accommodation requires completion of the above form.
Requests for Dining Accommodation will also normally necessitate that the student meet with the Dietician for Dining Services to review the student’s requirements and/or limitations.
